How do you trigger MCE in Vista Ultimate?

If you want the Media Center Edition rather than normal Vista, is that
something you have to choose at install time or do you set a preference
somewhere in regular Vista?

There is not a separate Vista Media Center edition. Media Center is just
another Microsoft application which is included in Vista Ultimate or Vista
Home Premium and you will see its Icon on your desktop after the
installation.
