Sharepoint and quickparts

When creating a form in word using the columns i added, the columns
show up in the document properties just fine, but when i goto add
quick parts then the check box, radio box and multi line items are
not
in the list for me to add. What am i doing wrong here? Heres a link
to
a photo of what i mean. Youll see the two check box categories but
theyre not listed in the quick parts.
