I have found an AVG removal tool that seems to remove the add in after AVG had been uninstalled (thank you Kaspersky help team) Go to:
http://www.avg.com/download-tools

Download & save the relevant file ( avgremoval.exe ) and run it. The problem vanished after the next re-boot and the avg add in is no longer listed in add in manager..

Nice one Trieu! I had no extend.dat file listed, but this fixed the problem.
