"Error! No table of contents entries found" if 4 levels-OK..

My MS Word file kept growing until I could not build 4 levels table of
contents any more - I get "Error! No table of contents entries found". I
still can build 2 levels table of contents. File size: 6+ Mb, 700+ pages, 2
tables of contents (one level 2 another level 4) took 100+ pages last time I
could build them
1) What should I do to be able again build 4 levels table of contents in MS
Word?
2) I use the MS Word file to collect snippets of text and organize them by
topic in a quite sophisticated hierarchy of categories. MS Word is obviously
is not the right tool for what I am doing. Could you please recommend to me
off the shelf software tool that allows 1) to store snippets of text, 2)
manually index them by topic (category), 3) easily add a category or
subcategory, 4) to regenerate a multilevel hierarchy categories (as MS Word
does), 5) do fast search both of the hierarchy of topics and the full
content, and 6) facility to move contents from MS Word doc to this new tool.
If there is more that one such tool I'd like to know about various of them.

Perhaps it would be easier if you just use the Document map instead of using
the TOC feature? View-->Document Map

I realize this post is from 2007, but I had tons of problems with this issue and found the solution by fluke so here it is:
Your document has accumulated too many hidden bookmarks.
Max number of bookmarks allowed per document can be as low as 16,379, depending on Word version.
To automatically remove them, run the VBA macro at http://support.microsoft.com/kb/184041
This works with Word 97 to Word 2007 (didn't try Word 2010).
Then recreate your table of contents.
