I have found a Document Map problem that did not occur in Word 2002. In Word
2007 the map display is not generated beyond a section break formatted with
an outline level above Body Text.
(Applying a higher outline level to section breaks is useful because it
makes them display in an Outline View restricted to that level.)
I can supply sample documents that demonstrate this problem.
A lower-level issue is that if the map does not fill the Document Map pane,
when the pane is resized the unfilled area is not refreshed correctly.

I'm not sure I follow what you are referring to. If I format a section break
using a Heading style it appears as a blank line in the Document Map. Can
you please provide some additional details, such as steps to reproduce what
you are describing?

I get the same as you when I start a new document. However with my existing
documents (originally created in Word 2000) the map display stops at the
first formatted break.
I guess if this is not a common problem I'll just live with it, and set a
suitable outline level for my section break style when I need to see breaks
in outline view.
Of course the refresh issue is a side-effect of the display problem.

Ok, well at least a workaround exists for this, but the underlying issue
remains. Perhaps this last bit of info here, along with the steps to
replicate the issue might help MS isolate the source of the issue.
I've analysed the differences between section breaks in my document and
found that those which "break" Document Map were those that were created
differently than the others that make Document Map happy. Poster "dimwight"
already identified this, so I'm simply expanding on the original descrption
here.
Here's the difference:
1. Those that make DM happy were created when the cursor is on a line with
"Body Text" or "Normal" style.
2. Those that make DM sad were created when the cursor is on a line that is
a Header 1, 2, 3, etc. For instance, I had an outline header I wanted to
start in a new section. So I put my cursor at the start of the header line
and created the new section. DM was sad and didn't want to include him - or
any following text - in the map.
Here's how I fixed my existing document. This approach works but one will
need to fiddle with document layout (landscape/portrait) and header/footers
as these will be reset as the section breaks are deleted.
3. Found a section break that makes DM sad. I started at top of document,
opened the "find/goto" window (Ctrl+F) and did a "go to" section break +1.
Then I did a Ctrl+PageDown to move down to the next section (Ctrl+PageUp)
will then go to the previous break). As I went to each section break I
looked to see if DM followed me (the section it likes will be highlighted on
the map). When the highlight went away I knew I found a section break that
made DM sad.
4. Removed the offending section break. This was done by clicking the
paragraph button (or keyboard combo Ctrl+Shift+ , SINGLE-clicking a section
break, and hitting the delete button on the keyboard.
5. Put my cursor at the start of the line where I wanted the new section
break.
6. Hit the left arrow key on the keyboard to back up my cursor one or two
characters. The number of characters is not as important as ensuring the
cursor is not on the line with a Header style. In some cases the previous
line was also formatted with a Header style so in those cases I created a
blank line with style "Body Text" or "Normal".
7. Created the section break while the cursor is on the line formatted with
style "Body Text" or "Normal".
8. Hit the delete key, as creating the section break in this manner prompts
MS Word 2007 to create an additional line for some reason. Who knows why
these things happen? You can sing to my cat if you like.
Document Map was happy after that and therefore so was I. And just in time
for donut break too. Yum.

John got me on the right path (thank you!!), but here's a fix that won't make you have to redo your page layouts:
With the Document Map turned on:
- Turn on the "show paragraph marks and other hidden formatting symbols" (looks like a weird backwards P, or use the shortcut Ctrl+*)
- Click on the last heading that appears properly in the Map, then navigate to the end of that section until you see the page break (double dotted line across the page; says "Page Break" or "Section Break" or whatever kind it is)
- Click in the left-hand margin to highlight the entire page break itself
- Apply the "Normal" style to the break
- You should see the next correctly-formatted section appear in the Map (i.e., the Map will fill in up to where there's another error)
- Repeat until the whole Map is filled in
