Customized tool bar buttons in Word 2003, won't stay

I have a client at the Division I take care of, that customizes his Word 2003 Toolbar with particular buttons. Once Word is closed and then restarted, the buttons go away. It will only keep the standard toolbar buttons. He has SP2 for Office installed and this is a new install of Office 2003.

I had a user with the same problem.
The problem is a corrupted .pip file.
