Ubuntu sends me "essential security upgrades" about twice a week, on
average. How secure, then, is Puppy Linux, which I love but which
hasn't been upgraded in many months? Can I trust it?

Puppy hasn't got as much software as ubuntu - and therefore there's
likely to be less security updates. But i wouldn't trust it to be secure
if it never gets updated at all. However, depending on how you managed
security risks, that might not matter. E.g., if you don't ever expose an
open ssh port to the internet, you may not need to worry about how
secure ssh is. Etc.

IIRC, doesn't Puppy run as root by default? If so, it's probably not
secure even when fully patched.
